مقدمة
يعتبر منظار القولون من أكثر المناظير انتشارا وتعتبر الثقوب الناتجة عن استخدام هذا المنظار تحديا للجراحين في ضوء صعوبة اتخاذ القرار الحاسم وتعاظم دور عمليات القولون التي قد يلجأ إليها الجراح
منهجية الدراسة
تم استقبال مجموعة من المرضى على مدى ثلاثة سنوات في وحده الجهاز الهضمي، وقسم الطوارئ، والوحدات الطبية بواسطة الفريق الجراحي المعد لتشخيص وعلاج ثقوب القولون الناشئة عن استخدام منظار القولون وبعد نقل المرضى، تم تقييم حالتهم في وحدة الجراحة وبدأ العلاج بتوفير راحة للأمعاء، وإعطاء مضادات حيوية ومحاليل بالوريد وكذلك علاجات مسكنة وبعد تقييم استجابة المرضى لهذا الأسلوب العلاجي بدقة تم تحليل النتيجة النهائية
النتائج
اشتملت هذه الدراسة على 38 مريضا، وقد تم إجراء التدخل الجراحي الطارئ لـ 35 مريضاً لدواعي جراحية مختلفة، بينما تم علاج الحالات الثلاث الباقية تحفظيا وقد أظهر هؤلاء المرضى درجات متباينة من التحسن
الخلاصة
يمكن توقع علاج بعض من الحالات المختارة من الثقوب الناتجة عن أجراء منظار القولون والتي قد تؤدي إلى إحداث التهاب برتوني نتيجة لتسرب البراز وهذا يجنب أجراء العمليات الجراحية الطارئة المتوسطة والكبرى للقولون Background
Colonoscopic procedures account for a substantial number of endoscopies performed
for various reasons Colonic perforations inflicted during colonoscopy pose a
challenge to the surgeons in the wake of difficult emergency decisions and major
colonic operations
Patients and Methods
Over a period of three years a group of patients was referred from the
Gastroenterology Unit emergency room andor medical units to the Surgical on call
team for the management of suspected colonoscopically induced colonic perforations
Following their transfer the patients were closely monitored in the surgical unit and
treatment was initiated by bowel rest IV antibiotics and fluids and analgesia
Patients’ response to this regime was meticulously scrutinized to analyze the final
outcome
Results
38 cases were incorporated in this series Out of these 35 were urgently operated due
to various surgical indications and the remaining 3 cases were managed
conservatively These patients made an uneventful recovery
Conclusion
In selected patients the colonoscopic perforations leading to localized fecal peritonitis
can be managed expectantly which can avoid moderate to major magnitude
emergency colonic operations
